SCREENING
The
following 100 question test will allow you to evaluate if your child
is likely to be suffering from undetected vision problems. Print out
this questionnaire and use the following rating scale to score your
child's performance, circling the number that best describes your
child.

If
you have circled a score that is marked with a double asterisk
(**), this means that a developmental
vision evaluation is suggested regardless of what your child's
total score may be. For instance, if your child "Frequently" or
"Always" gets headaches when reading, an evaluation is indicated
even if no other symptoms are present. Indeed, "A" students are more
likely to suffer from headaches than are children who avoid
schoolwork.
